New patient registration
We recommend that you are registered at the GP practice nearest to your home address to improve your access to medical services. If you are not registered with a GP practice or would like to change your registered practice to us, please visit the GP practice to collect the appropriate registration forms.
You will also need to bring the following items when you register with the surgery, either:
- photographic identification, such as a passport or driving license; and
- proof of address (this can be a letter that has your name and current address on and is dated within the last three months, such as a utility bill, bank statement, council tax letter or any other letter addressed to you).
In addition, if you are not a UK or EU citizen, we will need to see any relevant documentation from the Home Office as proof of your entitlement to access NHS services.
Registrations take up to seven working days to process. Please note that you can't see a GP until you have formally registered with the GP practice.
Registered patients at Northpoint Medical Practice must be resident within the boundaries of North Carr, Sutton, Orchard and University, Ings and Longhill.
All registered patients are assigned one named GP who is accountable for their care. For more information or to find your named GP, please contact reception.
